Monday – Friday: 8 am to 5 pm $10 per child Your child must meet one of the following criteria to receive TVFC (Texas Vaccines for Children)
1st Monday of Month: 8 am to 6 pm $15 per adult
  • Enrolled in Medicaid
  • Be WITHOUT health insurance
  • Be under insured (vaccines not covered)
American Indian or Alaskan Native Adults *19 or older with NO health insurance coverage

Vital Statistics is the registration, preparation, transcription, collection, compilation, and preservation of data pertaining to births, acknowledgements of paternity, deaths, fetal deaths (stillbirths), and related data. We issue certified death certificates on individuals that died in the city of Beaumont and Precincts I and VI.

    Certified Birth Certificate: $23
    Certified death certificate – first copy: $21
    Certified death certificate – each additional copy: $4

    Notary Affidavit: $6 / $1 each additional signatureThere is a $25 customer service charge on all returned checks. Accepted payments are: cash, personal check, cashier’s check, credit/debit card, or money order with proper identification. Make checks payable to: Beaumont Health Services Department. Personal checks must contain pre-printed name and address. Two-party checks and checks greater than the amount of purchase will not be accepted.

    All AMENDMENTS to birth certificates are processed through the State office in Austin and payable to the State Bureau of Vital Statistics.

  • Birth Records

    Birth records are confidential records for the first 75 years after filing and death records are confidential for the first 25 years. These records may only be issued to a properly qualified applicant upon submission of a written application that fully identifies the records.

  • Adoption Records

    Adoption records are confidential forever and may only be opened upon order of the court which granted the adoption. These records are opened at the State Bureau of Vital Statistics.

  • Acknowledgment of Paternity Records

    Paternity records are confidential. A certified copy may only be obtained through the State Bureau of Vital Statistics. Call the Vital Statistics office for more information.

  • Qualified Applicants

    A qualified applicant is the registrant, or an immediate family member (parent, grandparent, or sibling) either by blood or marriage, his or her guardian, or his or her legal agent or representative. Local, state, and federal law enforcement or governmental agencies and other persons may be designated as properly qualified applicants by demonstrating a direct and tangible interest in the record when the information in the record is necessary to implement a statutory provision or to protect a personal legal property right. This is demonstrated by written proof.
